Aim – to increase the effectiveness of the treatment of bronchial asthma patients who have had a COVID-19, taking into account the features of the pathogenesis of lung injury and clinical, functional characteristics. Methods – questionnaire, clinical, functional, radiological, laboratory, experimental, statistical. The clinical characteristics of the asthma patients who have had a COVID-19 are uncontrolled asthma ((12,2  0,4) ACT points), a high dyspnea level ((2,0  0,1) mMRC points), the presence of depression and a severe fatigue. The functional characteristics are: a decreased vital and forced vital capacity, which indicate restrictive lung disorders; a decreased FEV1 (65,7  2,1) % and MEF25 (32,8  2,4) %, which indicate the higher level of small bronchi obstruction; a decreased the respiratory muscles strength; decreased tolerance to physical activity. In experimental pulmonary emphysema the increased generation of MDA leads to the development of oxidative stress, violation of the functions of the antioxidant and antiperoxide systems, as evidenced by a decreased ceruloplasmin level and catalase activity in the blood. The mechanisms of biochemical disorders in asthma patients who have had COVID-19 and pulmonary emphysema are a decrease of the total protein content and signs of liver dysfunction. In asthma patients who have had a COVID-19 predominates the neutrophilic type of airway inflammation. An diagnostic algorithm of respiratory muscle dysfunction in patients with bronchial asthma after COVID-19 at the primary medical care level has been developed, which has high sensitivity (89,8 %) and specificity (81,8 %) and makes it possible to increase the accuracy of the diagnosis of respiratory muscle dysfunction by 18,3 % compared to the determination of only PImax and PEmax (from 70,0 % to 88,3 %).
