The aim of the work was to determine the relationship between cytogenetic and molecular genetic aberrations of blast cells with immunophenotypic markers, immunological, biochemical and clinical course features of acute leukemias with maturation. Clinical, cytological, cytochemical, immunophenotypic, cytogenetic, FISH, biological, statistical research methods were used. The frequency, features of clinical and hematological manifestations, course, morphological characteristics, immunophenotypic features of blast cells, the concentration and the nature and correlation degree of cytokine levels with indicators of neoplastic inflammation in AL with maturation were determined, risk groups and survival were analyzed. The variant of AL was confirmed according to the WHO classification (2016, 2022). Cytogenetic rearrangements of various kinds associated with AML M2 and AML M4 were found in 55 % and 50 % of patients respectively. The AML1/ETO gene was most often detected in AML M2 in 31 % of patients, the CBFB/MYH11 gene – in AML M4 in 14 % of patients. The statistical analysis of the patient survival according to the selected prognosis groups with cytogenetic markers was carried out, which allows choosing the most optimal treatment tactics. The results of the work were implemented in the SI "Institute of Blood Pathology and Transfusion Medicine of the National Academy of Medical Sciences of Ukraine" and MNE "Lviv Territorial Medical Union "Clinical Hospital for Planned Treatment, Rehabilitation and Palliative Care".
