The most common triggers were inflammatory processes of the genitals (51%), abortions (28%), thyroid pathology (26.3%), and genetic factors (44%). Based on the pathomorphological and immunohistochemical study of the endometrium of women with hyperproliferative pathology of the uterus, the pathogenetic mechanisms of the development of infertility were clarified, which consisted in changes in the receptivity of the endometrium, dysregulation of the processes of apoptosis and proliferation at the local level, which may be the cause of the disruption of the mechanism of blastocyst implantation. The histological and immunohistochemical features of endometrial pathomorphosis after hormone therapy with drugs of different mechanisms of action, which should be taken into account in women with infertility, have been established. Determination of epigenetic mechanisms of reproductive function disorders in women with hyperproliferative pathology of the uterus based on the study of miRNA status. The association of miRNA expression levels with the presence of infertility, the development of uterine leiomyoma, its size and the number of myomas nodes and the presence of concomitant pathology was established. Based on the obtained data, predictive miRNA panels were developed to determine sensitivity to hormonal drugs with different mechanisms of action. An algorithm for determining sensitivity to hormone therapy is scientifically substantiated and proposed, which allows for a differentiated approach to treatment. Based on the results of the work, a diagnostic algorithm and a differentiated personalized approach to the treatment of women with hyperproliferative syndrome and infertility were developed and implemented, which allows to increase its effectiveness and reduce the frequency of early pregnancy losses.
