Duration of observation in the main group: 2 years with 6-fold examination in the dynamics of treatment. Research methods: clinical and instrumental, assessment of carbohydrate metabolism and blood lipid spectrum, markers of inflammatory activation (interleukin-6, interleukin-18, soluble vascular cell adhesion molecule (sVCAM-1), monocyte chemoattractant protein-1 (MСP-1)), determination of the levels of circulating miRNA-126, -146a. It has been proven that the combination of CAD with T2DM leads to a decrease in the expression of circulating anti-inflammatory microRNAs-126 and 146a, which is accompanied by an increase in the activity of pro-inflammatory IL-6, IL-18, MCP-1. It was established that in patients with CAD with T2DM atheroprotective, anti-inflammatory effects of statins, sodium-dependent glucose cotransporter type 2 inhibitor empagliflozin may be partially mediated by increased expression of miRNA-126 and 146a. It was shown that in patients with CAD with diabetes, the anti-inflammatory effects of statins depend on the initial levels of miRNA-126 and, to a lesser extent, on miRNA-146a.
