The aim of the study: to investigate the activity of the end links of intracellular signaling cascades PI3K/Akt/mTOR/p70S6K and Ret/Ras/Raf/MEK/ERK in PBMC of patients with type 2 DM. Research methods and equipment: biochemical, immunoenzymatic, statistical, pipette dispensers, torsional and electronic scales, "Micro 22R" and "Hermle Z-300" centrifuges, "Stat Fax 3200" immunoenzymatic analyzer. Results: hyperactivation of the PI3K/Akt/mTOR/p70S6K signaling pathway was proven in patients with type 2 DM, which was assessed by determining the content of phosphorylated serine-threonine protein kinases – Akt (Ser473) and p70S6K (Thr389), as well as the proline-rich substrate Akt 40 kDa – PRAS40 (pT246) – a natural inhibitor of the mammalian target of rapamycin complex (mTORC) in PBMC. The degree of Akt (Ser473) phosphorylation reflects the activation of the mTORC2 complex, whereas p70S6K (Thr389) and PRAS40 (pT246) activation – that of mTORC1.Activation of protein kinases ERK 1 and 2, regulated by extracellular signals, was not observed in leukocytes of patients with type 2 DM, while it significantly increased in patients with type 1 DM or autoimmune thyroiditis. However, in patients with type 1 DM and autoimmune thyroiditis, the activation of ERK1/2 in leukocytes decreased to the control level, which can be explained by the competition between the two autoimmune processes for common signaling pathways.In patients with type 2 DM who received metformin monotherapy, a significantly lower content of phosphorylated p70S6K was found compared to the levels in patients who received other schemes of glucose-lowering therapy.
