The purpose of the work: to develop methods of diagnosis and surgical treatment of pathology of the thyroid gland, parathyroid gland and adrenal glands in order to improve the accuracy of diagnosis and minimize trauma and the number of specific postoperative complications. Research methods and equipment: biochemical, immunoenzymatic, statistical, cytological. Results: BRAF mutations were detected in 46.5% of patients with papillary carcinomas. The number of BRAF-positive cases in the Bethesda cytology groups was found to increase with increasing probability of diagnosis of carcinomas, starting with Bethesda IV. Benign thyroid nodules were all BRAF-negative. Among locally invasive papillary thyroid carcinomas, BRAF-negative cases predominated over noninvasive papillary carcinomas. Analysis of the results of treatment of radioiodine-resistant metastases of highly differentiated thyroid carcinomas showed that such factors as age 45 years and older, male gender, significant tumor size (>3 cm), multifocal lesions, capsular invasion, the presence of metastases can impair the effectiveness of the combined treatment and more often lead to recurrence of the metastatic process. Conclusions: In patients with highly differentiated carcinomas of the thyroid gland, who have an increased risk of developing radioiodine-resistant metastases, it is recommended to perform a total thyroidectomy and a compartment-oriented lymphatic dissection of the neck of the middle and lateral compartments of the neck. The presence of BRAFV600E gene mutation is a diagnostic marker of papillary thyroid carcinoma, its detection confirms the presence of malignancy. BRAF positive carcinomas were noted in 46.5% of observations.
