The aim of the work is to increase the efficiency of diagnostics of toxic-allergic reactions (TAR) of drug intolerance to anti-tuberculosis drugs (ATD) in patients with ТВ and to determine ways of their correction. Methods – clinical, immunological, statistical. It is established that the development of TAR is mainly associated with impaired function of the hepatobiliary system and the absence of correlation liaison of the level of serum Ige level with the level of tryptase, which indicates the genesis of other mechanisms of hypersensitivity to ATD. Identified markers of manifestations: lymphocytic coefficient LС, reflecting the activity of the immune system, lymphocytic-liver coefficient LPC, reflecting the violation of the balance of immune and hepatobiliary systems , toxic-allergic coefficient TACCH and lymphocytic-allergic coefficient LAC, reflecting the reaction of blood cells to specific ATD. With the help of these markers, a technology for diagnosing TAP to ATD in TB patients has been developed, the use of which can increase the accuracy of TAR diagnosis by 14.3% and algorithm of differential diagnosis of TAP to ATD, which makes it possible to increase the accuracy of differential diagnosis of TAP development by 36.84% and algorithm of laboratory differential diagnosis of TAR, AR and TR was developed, which allows to increase its accuracy by 18.2 %; increase the accuracy of the detection of hidden sensitization of LF to ATD by 53.6 % with the apostryor probability of clinical signs of hypersensitivity in 51.2 % of cases during treatment. Developed treatment and immunocorrection pathways in TB patients with clinical manifestations of TAR to ATD, which consist of additional prescribing Hepatomunil to the standard treatment TB, resulting in the therapeutic effect of treatment by reducing the stay of patients in hospital 1.5 times.
