It was established that the patients of the main group (271 patients who underwent left atrial plastic surgery) had better results in the remote period (86.4% of good and satisfactory results compared to 26.5% in the control group). Patient survival up to 7 years was 84.8% in the main group versus 73.5% in the control group. At the same time, sinus rhythm was maintained in 37.5% of patients in the main group, and only in 9.1% of patients in the control group. The level of freedom from thromboembolic complications within 7 years was 83.5% and 75.4%, respectively. In addition, in patients without correction of atriomegaly, the rate of severe thromboembolism in the long-term period was higher. In the structure of the causes of mortality in the remote period in the control group, cardiorespiratory failure and thromboembolic complications prevailed (total 77.8%). At the same time, in the main group, the same causes of remote mortality accounted for only 23.6%. 3 original methods of left atrial reduction were developed: arched (with a diameter of the left atrium up to 6.0 cm), triangular (with a diameter of 6.0-8.0 cm) and W-shaped (with a diameter of more than 8.0 cm). In general, the need to perform reduction of the left atrium and resection of its auricle in the presence of left atriomegaly has been proven.
