Objective: improving measures to prevent the development of irreversible organ damage and reduce the quality of life of children with JIA and SLE by controlling the activity of the disease, minimizing the manifestations of comorbid conditions and comorbidities, side effects of therapy. Methods: clinical, ultrasound, biochemical, immunological, assessment of quality of life, statistical. Results: 50.0% of children with SLE and JIA have irreversible heart damage with signs of myocardial dysfunction and maladaptive reactions to physical exertion, impaired external breathing of the restrictive type, kidney damage with reduced glomerular filtration rate, atherogenic changes in the lipid spectrum and system blood clotting The factors of their stabilization and progression are the long course of the disease with the preservation of the activity of the process. The overall level of health-related quality of life indicators in children with JIA and SLE is significantly reduced and closely related to activity indicators. With high indicators of physical and social functioning in children with JIA, the levels of emotional and school functioning turned out to be the most reduced. Novelty: it was proven for the first time that in children with SLE and JIA during long-term follow-up, persistent organ damage and metabolic disorders persist. It was established that the factors of their stabilization and progression are the long course of the disease with the preservation of immunoinflammatory activity. A significant decrease in quality of life indicators was revealed, regardless of the age of onset and duration of the disease.
