Registration Date 25-01-2024 Organization State institution "Institute of blood pathology and transfusion medicine of Nataional Academy of Medical scinces of Ukraine" popup.description2  During the reporting period, a retrospective analysis of the effectiveness of splenectomy was conducted in 31 patients with diffuse large B-cell lymphoma (DLBCL), 2 patients with hepatosplenic T-cell lymphoma (HSTCL), 6 patients with mantle cell lymphoma (MCL). Splenectomy was effective in 27 (87%) patients with DLBCL. All patients with abdominal syndrome experienced relief, body temperature normalized, reactive left-sided pleurisy disappeared, no ascitic fluid was observed on control ultrasound of abdominal organs. As a result of splenectomy, thrombocytopenia was eliminated in 17 (85%) patients, with leukopenia, the number of leukocytes was normalized; autoimmune hemolysis stopped in 3 (60%) patients. The number of atypical monoclonal lymphocytes remained unchanged. As a result of splenectomy in all patients with MCL, abdominal discomfort was eliminated, a large mass of tumor was removed, hemolysis stopped, hypersplenism was eliminated, hemoglobin and platelet levels were normalized or increased, histologically and immunohistochemically the variant of lymphoma was clarified or confirmed. When analyzing the long-term results of splenectomy in patients with MCL, it was established that the patients lived 49, 70, 140, 280 months after the operation, and all of them started cytostatic therapy courses after 2-12 months, which after splenectomy turned out to be significantly more effective. The effectiveness of splenectomy in hepatosplenic T-cell lymphoma is low, and remission after surgery is short-lived.
