The purpose of the work: to improve the results of treatment of victims of dermal burns by developing a diagnostic algorithm for predicting the course of the wound process, improving and rationally combining modern methods of surgical interventions, transfusion therapy schemes to reduce the risk of developing septic complications of the course of the disease in thermal trauma. Research methods: clinical, cytological, cytochemical, hematological, immunological. The developed program of surgical treatment of patients with dermal and deep burns contributes to the reduction of toxic damage to peripheral blood phagocytes, which is manifested by an increase in indicators of functional activity of neutrophil granulocytes, indicators of myeloperoxidase activity and a decrease in the content of cells with apoptotic changes in patients with burns in the stage of septicotoxemia relative to indicators of toxemia. Application of the developed algorithm for surgical treatment of burns contributed to an increase in the content of monocytes and lymphocytes in the area of ​​the wound defect, which leads to the optimization of reparative processes in the area of ​​the burn wound. There is an established tendency to optimize the functioning of monocytes. The use of the proposed treatment algorithm in complex surgical treatment leads to a decrease in the activity of reactions associated with oxidant stress, which reduces the manifestations of endothelial dysfunction, and optimization of interaction in the cascade cytokine interaction system, reduces the activity of pro-inflammatory cytokines and regulates the development of the inflammatory reaction both in the zone of thermal damage and body as a whole.
