Purpose: improvement of diagnosis and identification of prognostic markers of adverse course of myocarditis. Methods and equipment: study of immune status indicators using the IEMS Labsystems immunoenzyme analyzer, echocardiographic indicators on the ultrasound device Aplio Artida SSH - 880 CV (Japan), the results of daily ECG monitoring on the Philips Digitrack TM-plus 3100A device (Hungary), cardiac magnetic resonance (CMR) on the Toshiba Vantage Titan HSR 1.5 Tesla machine (Japan). Based on the results of the research, it was established that the high activity of immunopathological reactions in the 1st month after the onset of myocarditis is associated with long-term persistence of the inflammatory process in the myocardium, dilatation and decrease in the contractility of the left ventricle. A set of instrumental indicators obtained within the first month from the onset of myocarditis, which can serve as predictors of cardiovascular events during the next 12 months, was determined: left ventricular ejection fraction ≤ 30%; index of longitudinal global systolic strain ≤ 7.0%; the presence of paroxysms of unstable ventricular tachycardia; the presence of inflammatory changes in ≥6 segments and delayed contrast enhancement in ≥5 segments of the left ventricle. With the help of discriminant analysis, mathematical models were created, the use of which within the first month from the onset of myocarditis allows predicting the adverse course of the disease with long-term (at least 12 months) persistence of heart failure and paroxysms of unstable ventricular tachycardia.
