Research Objective: To analyze various treatment programs and investigate potential laboratory markers for ankylosing spondyloarthritis (AxSpA) and systemic lupus erythematosus (SLE), study the properties of adipose tissue mesenchymal stem cells (MSCs) in patients with SLE and AxSpA, and evaluate the effects of lipopolysaccharide (LPS) and dexamethasone on their activity. Methods of Study: Clinical; Laboratory studies: endothelin-1 (ET-1), ferritin, interleukin-2, serum amyloid A (SAA), cortisol, leptin, and antinuclear antibodies; Radiological: cervical and lumbar spine and sacroiliac joints X-ray; Cultural; Statistical: Data analysis using Statistica 10.0 and R-studio. Results. Osteoproliferative changes (syndesmophytes) in AxSpA have been found to be associated with decreased BMD. Reduced BMD and osteoproliferative changes share common risk factors: the activity and duration of AS, smoking, and the use of glucocorticoids. It was established, that in patients with AxSpA, weight gain of 1 kg resulted in a less pronounced increase in leptin concentration compared to the control group. Cortisol level in AxSpA patients did not significantly differ from the control group, was not associated with disease activity, and therefore cannot be used as a laboratory marker for AxSpA. Mycophenolate mofetil demonstrated the best efficacy and safety profile in SLE, making it a priority treatment option for patients with a significant comorbid background. Markers of high SLE activity were identified as: SAA levels above 35.75 mg/L (sensitivity 83.3%; specificity 68.8%), ET-1 levels above 4.14 pg/mL (sensitivity 58.3%; specificity 85.7%). The growth kinetics of adipose tissue MSCs in patients with SLE and AxSpA were significantly slowed. While LPS substantially stimulated growth, dexamethasone had no effect on growth kinetics but demonstrated the ability to negate the effect of LPS.
