The aim of the work was to determine the relationship between cytogenetic and molecular genetic aberrations of blast cells with immunophenotypic markers, immunological, biochemical and clinical course features of acute leukemias with maturation. Clinical, cytological, cytochemical, immunophenotypic, cytogenetic, FISH, biological, statistical research methods were used. The analysis of clinical and hematological features, morphological, immunophenotypic, cytogenetic and functional features of blast cells was conducted. The concentration of cytokines was investigated. The nature and degree of correlation of their level depending on the ratio of the absolute number of neutrophils to lymphocytes, the severity of anemia and thrombocytopenia were determined in AL with maturation. Risk groups were determined and survival was analyzed. The variant of AL was confirmed according to the WHO classification (2022). Cytogenetic rearrangements of various kinds associated with AML were found: in 58 % of AML M2 – AML1/ETO gene, t(8;21)(q22;q22), -Y, 3q21q26 rearrangements, del(9)(q22); in 74 % of AML M3 – PML/RARа gene, t(15;17)(q22;q21), tetrasomy 8; in 50 % of AML M4 – CBFB/MYH11 gene, inv(16)(p13q22), -7, 11q23/MLL rearrangements; in 73 % of AML M5 – del(11)(q23), BCR/ABL gene, t(9;22)(q34;q11), +8. The statistical analysis of the patient survival according to the selected prognosis groups with cytogenetic markers was carried out, which allows choosing the most optimal treatment tactics. The results of the work were implemented in the SI "Institute of Blood Pathology and Transfusion Medicine of the National Academy of Medical Sciences of Ukraine" and MNE "Lviv Territorial Medical Union "Clinical Hospital for Planned Treatment, Rehabilitation and Palliative Care".
