The objective of the work is to determine the mechanisms of the modifying effect of COVID-19 on radiation-induced effects in the critical systems of the body of those affected by the Chornobyl accident to optimize the diagnosis and treatment of the long-term consequences of coronavirus disease. Results. The clean-up workers showed pronounced disorders of the respiratory, cardiovascular and nervous systems. In the clean-up workers, the consequences of COVID-19 can be considered a violation of the heart rhythm such as ventricular extrasystoles and atrial fibrillation, a change in the structural and functional state of the myocardium with a decrease in the ejection fraction, an increase in the volume of the right chambers of the heart. The clean-up workers have bilateral cerebral organic damage with a violation of information and cognitive processes of the brain in the dominant and subdominant hemispheres. The clean-up workers who died from COVID-19 had a severe chronic disease or several diseases that aggravated the course of the coronavirus disease and each other and contributed to the development of fatal outcomes. The markers of COVID-19-associated immune system disorders are the indicators of granulocytes, B- and T-cells, CD4+ T-cells expressing activation markers CD25 and CD27, and CD56+ NKT, cytokine-producing lymphocytes and the level of pro- and anti-inflammatory cytokines and NF-κβ. Activation of effector T-cells, NK and NKT, changes in cytokine signaling due to COVID-19 lead to disturbances in immune reactivity and the development of a systemic mild inflammatory response, which can cause the progression of chronic pathology. An algorithm of actions has been developed for damage to the respiratory and cardiovascular systems after COVID-19 infection in persons who suffered from the Chornobyl catastrophe.
