Komissarenko Institute of Endocrinology and Metabolism of the National academy of medical sciences of Ukraine" popup.description1 Assessment of the risks of severe and complicated course of COVID-19 in patients with diabetes mellitus (DM) popup.description2 Databases were formed for 794 patients with diabetes mellitus who had COVID-19. Among them are pro-inflammatory factors (IL-6, IL-17A, IL-33, IL-22, NF-κB), factors that complicate the course of CVD include LP(a), apolipoprotein B, oxidized low-density lipoproteins, IL-22, IL-17A, and fibrosis factor TGF-β1. Rare factors GM-CSF, MCP-1, and MIP-1a were identified. Changes in the content of a number of critical health factors in different age groups were studied. It has been shown that insulin, biguanide, SGLT-2 inhibitors, and their combination effectively reduce the amount of oxLDL and other pro-inflammatory factors in the blood of patients who have had COVID-19, and therefore the risk of atherosclerosis. Algorithms for diabetes treatment in post-COVID patients were updated. First, the identified biomarkers - ApoA1, ApoB, oxLDL, Lp(a), IL-17A, key inflammatory cytokines, as well as markers of biological age (telomere length, leukocyte mtDNA level) can be included in expanded laboratory monitoring panels for people with diabetes, with acute respiratory infections, and in the post-COVID period. Their application will improve early detection of susceptibility to microvascular and macrovascular lesions. Second, the results can be used to improve clinical recommendations for the management of patients with diabetes and COVID-19, in particular by clarifying risk criteria, stratification algorithms, and pharmacological correction of lipids and inflammatory disorders. The established relationships between glycemic status, cytokine profile, and atherogenic changes make it possible to form personalized treatment and monitoring regimens. Integration of telomere and mitochondrial markers into the medical monitoring system will allow assessing the biological age of patients and predicting the risks of progression of chronic complications.
