Aim – to determine modern clinico-morphological forms of pulmonary tuberculosis and the specific features of pleural tuberculosis based on microbiological and molecular-genetic findings. Methods – cytological, histological, histochemical, microbiological, molecular-genetic, radiological, statistic. During the study period, 2198 patients were hospitalized with pulmonary TB and 124 – with pleural TB, representing an average annual hospitalization rate of 12,0 %. The mean age of pulmonary TB patients was 39,4 years, male predominance; and 40,2 years for pleural TB, no sex difference. All cases were bacteriologically confirmed, drug-resistant strains predominated in pulmonary TB, whereas drug-sensitive forms were more frequent in pleural TB. The disease duration at admission did not exceed one year in 66,5 % of pulmonary TB and 97,6 % of pleural TB cases. Relapses accounted for approximately 15,0 % of pulmonary TB cases. Among pulmonary TB cases, infiltrative and disseminated forms predominated. Tuberculomas and fibro-cavernous TB accounted for 4-7,0 % of cases, while other forms were sporadic. A total of 241 surgical interventions were performed for pulmonary TB, with 239 surgical specimens obtained. A marked conversion of clinically active forms of pulmonary TB into other forms was observed, in particular – tuberculomas (the proportion of which doubled) and fibro-cavernous TB, representing manifestations of regress of the specific process in such cases, while infiltrative and disseminated TB decreased fivefold. The diagnostic efficiency of histological biopsy examination was approximately twice that of cytology, averaging 80,8 % for pulmonary TB. In cases of combined pulmonary-pleural involvement, the diagnostic accuracy of pleural biopsy histology reached 96,9%.
