Aim – to increase the effectiveness of treatment of patients with the complicated course of the bronchial asthma severe exacerbations in military personnel and civilians injured as a result of military operations. Methods – questionnaire, clinical, functional, radiological, experimental, biochemistry, statistical. Clinical and functional characteristics of severe BA exacerbations in military personnel and civilians injured as a result of military operations are uncontrolled asthma (according to ACT (10,7 ± 0,6) and (12,5 ± 0,5) points and ACQ (2,9 ± 0,1) and (2,7 ± 0,1) points, respectively), high level of dyspnoea (according to mMRC (2,6 ± 0,1) and (2,3 ± 0,1) points, respectively), decreased vital and forced vital capacity; decreased FEV1 to (60,2 ± 2,4) % in military personnel and (63,2 ± 2,7) % in civilians, severe bronchial obstruction, at the level of small bronchi also, decreased tolerance to physical activity. According to the X-ray examination patients with severe BA exacerbations have concomitant respiratory diseases: bronchiolitis, pneumonia, fibrosis, sarcoidosis, pneumoconiosis. It was established that most patients had comorbidity, mainly allergic and cardiovascular diseases. Among other pulmonary diseases in addition to BA, concomitant COPD and pneumonia were observed. In experimental pulmonary emphysema, the activity of glutathione metabolism enzymes is decreased: glutathione-reductase, glutathione-S-transferase and glutathione-peroxidase in the blood. An algorithm for diagnosing of the complicated course of the bronchial asthma severe exacerbations in military personnel and civilians injured as a result of military operations has been developed, the use of which allows to increase the accuracy of bronchiolitis diagnosis by 23,5 %.
