Registration Date 28-01-2026 Organization State Institution "LV Gromashevsky Institute of Epidemiology and Infectious Diseases of the National Academy of Medical Sciences of Ukraine" popup.description1 On the basis of molecular and epidemiological studies, to study the problem of the formation of HIV resistance to antiretroviral drugs in the conditions of large-scale military operations in Ukraine popup.description2 For the first time in Ukraine, factors associated with an increased risk of HIV drug resistance formation were identified using multivariate regression analysis. It was found that the risk of acquired HIV drug resistance significantly increases with the duration of antiretroviral therapy (ART) for more than 24 months, if the patient has active tuberculosis and the level of CD4 lymphocytes <200 cells/μl. It was established that most often (78.7%) in patients on dolutegravir (DTG)-containing ART regimens, HIV drug resistance mutations to efavirenz (EFV) - a drug of the class of non-nucleoside reverse transcriptase inhibitors are detected. New data were obtained on the impact of the migration activity of the population caused by military actions on the epidemic process of HIV infection. It was established that in Ukraine, subtype A HIV-1 still prevails, however, its share in the subtype structure of HIV, as well as the share of subtype B HIV-1, are gradually decreasing due to the increase in recombinant forms CRF02_AG, CRF03_AB, combined variants B/A6. For the first time in Ukraine, it was established that military actions can increase the risk of the spread of resistant forms of HIV, since there were trends towards an increase in cases of patients refusing ART in order to reduce CD4 lymphocytes, worsen their well-being and avoid conscription into the ranks of the Armed Forces of Ukraine. It is shown that annual monitoring of Early Warning Indicators (EWI) of HIV drug resistance is an important element of epidemiological surveillance of HIV infection, which helps to identify factors that have a negative impact on the risk of developing HIV drug resistance.
