Yanovskyi National academy of medical sciences of Ukraine" popup.description1 To increase the effectiveness of surgical treatment of patients with pulmonary-pleural complications that arose as a result of hostilities and martial law in Ukraine popup.description2 Object – 115 patients with intrathoracic pathology of the lungs and pleura, with complications because of hostilities and martial law in Ukraine, who required surgical treatment at the stage of highly specialized care. Aim – to increase the effectiveness of surgical treatment of patients with pulmonary-pleural complications that arose because of hostilities and martial law in Ukraine. Methods and equipment – clinical, surgical, radiological, laboratory; computer tomography, videothoracoscopic complex, videofibrobronchoscopic complex, endobronchial bronchooccluder, bedside pleuroaspirator. New knowledge was obtained regarding the features, types, and mechanisms of formation of pulmonary-pleural complications as a result of hostilities and martial law in Ukraine, which require surgical treatment at the stage of highly specialized care, including pulmonary and pleural tuberculosis. It was established that the basis of pulmonary-pleural complications, the treatment of which is not provided with proper results at all levels of medical care, except for highly specialized, is the unfavorable course of the consequences of intrathoracic combatant and military trauma, or the formation or exacerbation of existing diseases under the influence of factors of being in a military environment. Most often, long-term, poorly curable infiltrative and destructive lesions or their combination were observed (77.3%), which were often accompanied by bronchopleural, bronchopleurothoracic communication (28.1%), pleurothoracic fistulas (7.5%).The etiopathogenetic factor was unknown or required clarification in the vast majority of patients 89.7%, while almost a third in our conditions were found to have various types of tuberculous lesions. Based on the results of the examination obtained in our conditions, a set of personalized treatment measures was applied, which allowed achieving satisfactory treatment results in almost 98% of treated patients.
