The work was carried out on the basis of medical institutions of the military medicine system and covers various stages of medical care - from advanced surgical groups to specialized hospitals. The study analyzed the clinical course of traumatic illness in wounded with massive blood loss, the features of the development of hemorrhagic shock and trauma-induced coagulopathy, as well as their influence on the choice of anesthetic support methods and intensive care tactics. Special attention is paid to the role of the anesthesiologist as a key member of the DCR multidisciplinary team, responsible for maintaining adequate perfusion, minimizing secondary injuries and optimizing conditions for surgical control of bleeding. The paper presents a comparative analysis of various strategies of infusion and transfusion therapy, including restrictive resuscitation, the use of blood components in physiological ratios, early correction of hypocalcemia and coagulopathy, and the use of vasopressor support in patients with unstable hemodynamics. The impact of the selected anesthetic tactics on the indicators of systemic hemodynamics, metabolic status, and the course of the postoperative period was analyzed. The results obtained indicate that the integration of DCR principles into anesthetic support algorithms allows reducing the volume of non-physiological infusion, reducing the severity of metabolic disorders, and stabilizing hemodynamics in the most critical period of treatment. It was established that the choice of anesthetics and anesthesia methods in wounded patients with hemorrhagic shock should be based not only on the depth of anesthesia, but also on their effect on systemic vascular tone.
