For normative parameters it is selected 102 able-bodied boys who had no pathology of external generative organs. It is learned clinicolaboratory alterations in children with pathology of lowering testes before, in the early and remote terms after orchiopexy. It was carried out general clinical inspection, somatometry, genitometry; determination of a hormones level - testosteron, luteinizing hormone, follice-stimulating hormone, T3, thyroxine and thyroid-stimulating hormone; estimation of immune reactivity according to indexs of cellular, humoral and cytophagous immunodefence, analysis of spermogrames, ultrasonic inspection of organs of a scrotum with Dopler, cytogenetic inspection, definition of the basic enzymes of energy metabolism - hexokinases, lactate dehydrogenase, glucose-6-phosphate dehydrogenase, sukchynate dehydrogenase and sexual and psychosexual developments according to biographical data. In boys with retained testes established alterations of genito- and somatometrical, biochemical, hormonal and immunological indexs, blood stream in testes, spermogram, sexual and psychosexual development. They accrue with the age of patients and at a bilateral pathology. In the early postoperative terms autoimmune processes are activated and the blood stream in the reduced testes is worsened. Clinical it is displaid by aseptic autoimmune orchiepididymitis - an edema of gonad. The received data testify to expediency of comprehensive approach for studying various etiopathogenetical factors of lesion of retained and reduced testes in boys. It is developed references on complex treatment of children with various forms of retained testes. For the first time is offered the efficient method of the prevention of gonads hypoplasia - overlapping of regional ilio-hypogastric-inguinal blockages and organoprotection before orchiopexy in boys. For the first time it is in complex examined children with a pseudocryptorchism. This fault is necessary for considering as disease - pathology of lowering of testes, with application of all necessary medical measures under the prevention of andrological and sexological complications. Tactics of expectation and simple observation is dangerous in the plan for development of serious problems concerning fertility. It is marked efficacy of lowinvasive methods of surgical treatment at various anatomical variants of transabdominal cryptorchism in children of all age-grades.
