Goal: The determining of structure and priority risk factors, target-organs damages, concomitant diseases influenced on EH follow-up in survivors after Chernobyl disaster for creating the guidelines for management of hypertensive survivors after Chernobyl disaster according to risc stratification. Methods: clinical, analysis of irradiation influence as well as others risk factors, daily blood pressure (BP) monitoring, electrocardiography, dopplerechocardiography, bicycle ergometry, serum blood investigations. Results: structure and priority risk factors, association diseases influenced on EH follow-up in survivors after Chernobyl disaster were determined. For clean-up workers after Chernobyl disaster mild and moderate EH was regular as well as increasing of BP variability, insufficient night BP lowering. Priority meaning of irradiated doze more 25 cSv, integral score of negative recollection Chernobyl disaster about more 10 and such traditional risk factors as unfavorable heredity, obesity, serum cholesterol high level for total risk estimation was evidenced. Association of priority risk factors due to Chernobyl disaster and life stile risk factors, metabolic changes sufficiently influenced on total risk. Among target-organs damages due to EH in clean-up workers after Chernobyl disaster left ventricle hypertrophy, carotid arteries intima-media complex thickening were determined as priorities. Microalbuminuria was discovered in 13.3%. Eyes retina arteries narrowing as a result of insufficient diagnostic meaning was excluded as factor of total risk estimation. In 51.7% clean-up workers after Chernobyl disaster moderate EH was associated with such disorders as coronary heart disease, heart failure. In such patients increasing of BP variability, insufficient night BP lowering were regular. Based on received data total risk stratification protocol was created. According to the protocol total risk was estimated as well asfollow-up groups were selected. In 92.5% mild and moderate EH patients total risk was estimated as high and very high. Majority of risk factors in clean-up workers after Chernobyl disaster might be corrected that substantiated possibility of influence on EH follow-up. Efficacy: Based on determining of priority risk factors, target-organs damages, associative cardiovascular diseases in clean-up workers after Chernobyl disaster with EH total risk stratification protocol was created. The protocol can give possibility of classification of complications total risk levels. Differenced groups according to risk level for consequence observation were selected. Within next phases it will permit to create pathogenetic measurements of complications prevention for clean-up workers after Chernobyl disaster with EH.
