It was determined that I/D polymorphism of the ACE gene influences significantly on formation of the salt sensitivity, clinical course of the disease, 24-hour profile of BP and hemodynamic indexes. It was established, that presence of D-allele of the ACE gene is accompanied with negative changes of investigated parameters, especially in the patients with sodium-sencitive arterial hypertension (SSAH). The possibility of SSAH diagnostics basing on the results of 24-hour BP monitoring is substantiated. The negative influence of the gene ACE D-allele and genotype DD on the effectiveness of antihypertensive treatment is shown. In patients with metabolic syndrome X D-allele of the ACE-gene is shown to be assotiated with more severe course of the disease according to the results of 24-hour BP monitoring, indexes of platelet aggregation and lipid metabolism. As a result of the study, differentiated schemes for prescription of antihypertensive treatment to patients with essential hypertension are worked out, according to genetic and associated clinical and hemodynamic characteristics of the patient.
