Goal: The determining of etiological and clinical structure, priorities risk factors of heart arrhythmias in clean-up workers after Chernobyl disaster, the creating groups of patient with the most prevailing HA, the creating of diagnostic program for their stepwise investigation. Methods: clinical, analysis of irradiation influence as well as others risk factors, Holter electrocardiography monitoring, dopplerechocardiography. Results: The HA and changes of their frequency during 20 years and their etiological and clinical structure were determined in 742 clean-up workers after Chernobyl disaster. The frequency of HA was increasing during 20 years from 20 % to 40 %. The coronary heart disease, hypertetsion and theirassociation were prevalent in etiological structure of HA. The frequency of HA in other heart diseases was 4,6 %, as well as idiopathic - 4,3 %. The analysis of risk factors before HA was registered the association of Chernobyl catastrophe factors (exposure to >25 sSv, working by shift method, negative recollections about Chernobyl disaster) and tobacco smoking, hereditary defects, hypertension, left ventricular hypertrophy, hypercholesterolaemia. The HA quantity in main group were two times more then control group. The frequency and duration of myocardium ischemia episodes in patients with HA were more then in compared and control group. Number of ventricular ectopies correlated with duration of myocardium ischemia, left ventriculus wall thickness and myocardium mass index. The ventricular heart arrhythmias were associated with increase myocardium mass index and diastolic dysfunction. Efficacy: The priorities of etiological and clinical HA structure, previous factors of HA, its course peculiarities of clean-up workers after Chernobyl disaster were determined. The diagnostic program for their stepwise investigation will conductive to increasing the efficacy of diagnostic, creating database for subsequent stages of investigation.
