International and national scientific methodological sources concerning elaboration and application of method of topometric preparation and planning of irradiation of head and neck and thoracic tumors by photons from linear accelerators were analyzed. The structure and technical supply of leading radiology departments in Ukraine were examined and their needs regarding the organizational and technical modernization and implementation of optimized protocols of planning of radiotherapy by photon linear accelerators were defined. Methods of pre-irradiation preparation of patients with lung and prostate cancer were elaborated. Novelty. For the first time the necessity of a state program of gradual multipart modernization of radiotherapy centers in Ukraine was substantiated, that included a formation of leading inter-regional centres of competence and peripheral centers for public service. Considering the established conditions of technical supply and methodological approaches to the topometric preparation, immobilization and planning of radiotherapy in leading cancer centers across the world, the most rational methods of pre-radiation preparation for head and neck and thoracic tumors were defined for Ukraine. New methods of pre-radiation preparation of patients with lung or prostate cancer were suggested. Implementation effectiveness. Elaborated methods of pre-radiation preparation of patients with lung or prostate cancer allows to increase the accuracy of irradiation volume estimates and to decrease the damage of surrounding normal tissues.
