STRUCTURE EPILEPSY, NEUROPSYCHOLOGICAL DISTURBANCES, MULTILOBAR RESECTIONS, CALLOSOTOMY, FUNCTIONAL HEMISHEROTOMY Object of study: multifocal epilepsy, epileptic seizures Subject of study: clinical manifestations of epilepsy, cognitive and psychoemotional disorders, differentiated neurosurgical interventions. Aim of study: to improve the results of surgical treatment of multifocal epilepsy due to a differentiated approach to the choice of surgical intervention depending on the clinical manifestations of the disease and structural and functional changes of the brain. Factors that determine the severity and prognosis of the course of epilepsy have been established. Unfavourable factors include the early onset of epilepsy, a high frequency of epileptic seizures resistant to antiepileptic therapy, progressive epileptic encephalopathy, and the presence of bilateral epileptic paroxysmal brain activity. Established neuropsychological criteria for prognosis regarding memory-intellectual decline. Unfavourable factors include the presence of localization of epileptic or epileptic foci in the dominant hemisphere and the duration of the disease. It has been established that the best results of surgical treatment of multifocal epilepsy occur in case of localization of epileptic foci or diffuse epileptic focus within one hemisphere. In such cases, isolated multilobar resections or in combination with disconnection operations lead to a complete control of epileptic seizures in the vast majority of patients.
